Unless exempt, all
students are required to be immunized against these diseases: Diphtheria,
Pertussis (Whooping Cough), Polio, Measles, Mumps and Rubella. Beginning
in the 1999-2000 school year all kindergarten students will be required
to be immunized against Hepatitis B.
The required minimum doses of vaccine to achieve protection against these
diseases are as follows:
DIPHTHERIA, PERTUSSIS, TETANUS (DPT, DtaP, Td, DT)
A minimum of 4 doses are required.
5 doses are required if the fourth dose was given before the fourth birthday.
POLIO (IVP)
3 doses - of all IVP.
A minimum of 3 doses are required.
A 4th dose is required if the third dose was given before the fourth birthday.
MMR (MEASLES, MUMPS, RUBELLA)
These three vaccines are usually administered as a combined MMR vaccine.
Beginning in the 1999-2000 school year all kindergarten students must
have 2 doses of MMR vaccine.
The first dose must have been given on or after the first birthday.
The second dose must have been received at least 28 days following the
first dose.
All 7th through 12th graders must have evidence of having received a second
dose of MMR.
HEPATITUS B
3 doses are required.
The second dose must have been received at least 28 days following the
first dose, and the third dose at least three months after the second.
If the third dose was given before six months of age a fourth dose is
required.
If your doctor says immunization would be harmful to your child, your
child can be exempted from these immunizations with a signed statement
from your doctor.
If you object to immunization for good cause, for example religious conviction
or philosophical differences, your child can be exempted from these immunizations
with a signed statement from a parent.
The state of Ohio mandates that school children not in compliance be excluded
from school fifteen calendar days after admission.
You may obtain immunizations
at your family physician's office or at the Huron County General Health
District immunizations clinics throughout Huron County.

Second Wednesday of the month - Norwalk - Health Department 9:00 a.m.

Third Wednesday of the month - Willard - YMCA

Third Thursday of the month - Wakeman - Area Family Care Center 2:00 p.m.
